Audition Form
Click on the image above to fill out the Audition Form in your desktop browser and select the printer icon (in the upper right-hand corner) to print at home. If you prefer to hand write your information, click on the image and select the printer icon to print a blank form. This must be done on a desktop, not a mobile device. If you do not have a printer, please arrive early to complete a form available at the welcome table.
